I received the fans, however the model I received was not PLA09215B12H as listed, it was a slower model called T129215SU. The fans I was supposed to receive and advertised as such spins up to 3500 RPM at .55V. The fans I received were only advertised up to 3000 RPM with .50V. I don't know if it's worth the hassle returning.

The fans have arrived in a small sturdy cardboard box, fans themselves were nicely wrapped in a bubble wrap. Fans were new as advertised, and appear to be exactly the same as the original ones. Wire lengths and connectors also match perfectly. I've installed them on the card, and they work without any issues. I'm pretty satisfied overall.
